Services
Provide companionship (hourly and live-in from respite to long-term), friendship and emotional support. Our services offer: Post-Hospital Recovery; Case Management; Transportation (i.e., Dr. appointments); Medication Reminders; Meal Prep and Plan; Light Housekeeping; Home Safety; Pet Assist; Improve Mobility and Stimulate Mental Awareness. Nurse designed and managed client care plan and procedures.
The minimum hour requirement for shift companionship is 4 hours, 2 days a week. We also provide overnight shifts (8-12 hours) and Live-In companions.
In addition, Helping Hearts at Home offer drop-in visits and social phone calls to help acclimate our clients to having help at home.

Areas of SCREEN review: Current living arrangements, current caregiving arrangements, informal support systems and mental status. Plans for continuing care determined and information given regarding the range of services available to the client.

Our home care services include performing H/C PRI and SCREENs for individuals who require placement or continued stay in residential health care facilities in the state of New York. We will travel to out of state to perform services as well.
